Rental Authorization Form

This form is to authorize CSI Rentals to charge your credit card and release your rental order to a third party in the event that the credit card member will not be present at pick up. - or for first time deliveries and out of state shipments 

 Following is required along with authorization form

► Copy of credit card (both sides)   

► Proof of address (bank statement, utility bill etc) - Card billing address must be in USA  

► USA Government Issued Photo ID for Renter And For Third party (US driver's license or US passport)

International Renters Please Note: If you are not a US resident the credit card member must supply credit card and passport "in person" prior to rental pick up or delivery, Authorization form is only for US residents
